"I was assigned a task by a supervisor who had a propensity to get stressed out when a deadline was approaching. They needed extra help and recruited me. They tended to be ambiguous in their communication, which typically required extra effort on my part to ensure we were on the same page. On this occasion, there were a lot of distractions, and it was clear the questions I asked them increased their stress levels, so I decided to move forward with the information I had on hand. But there was a communication breakdown, and the instructions weren't as clear as I believed they were. So, in the end, it required a lot of overtime and rework to accomplish the work I agreed to. I should have clarified at the forefront and knew better at the time. Not a mistake I'd ever make again."
