"When I was a trainee, I was asked to collect data on our clients, and I spent two days doing that. When I submitted my final report, I realized I had made a mistake and collected the incorrect data. Although I was paying attention, there was a part in which I could not hear the instructions given clearly. Since I was new, I hesitated to ask questions. My supervisor called me into her office and pointed out my error. I embarrassingly admitted my mistake, apologized, and collected the correct information.
