Nurse aides, nurses, and nurse leaders must conduct effective end-of-shift reporting to maintain quality patient care. Describe your process and communication as you wrap up your shift and hand over responsibilities to the next charge nurse on duty.

"All floor nurses and aides provide an end-of-shift report to the employee taking their place. This report includes details about patients they have cared for, changes in the patient's status, medication updates, fall risk changes, etc. As the charge nurse, I do something similar by providing an overview for the next charge nurse and including the same details and any personnel updates (like callouts) they may need. I would also give updates on compliance issues, medication errors, patient falls, etc. I document and communicate my updates and information and allow plenty of time for questions."

"I always complete all required documentation as I complete my shift. I wrap up my tasks and responsibilities as best as time allows, and if any outstanding tasks or reports need to be done, I make a note and share that with the next charge nurse immediately. When I do my handoff, I also update them on new admissions, upcoming discharges, changes in patient status, personnel issues, reportables, etc. If there are any questions, I answer them and assist in any way I can."
I encourage staff to use report sheets to keep the shift change and patient hand-offs concise and containing pertinent information. I created a patient report sheet that I use during my shift to take notes on important things on patients that I want to relay at shift change. This keeps the report organized and reduces the amount of information that is not communicated.
