What do you know about the risks of pharmacy malpractice or pharmacist negligence?

"Yes, I am aware of cases of malpractice in the pharmaceutical field, and it can be disheartening to hear these stories. Although I would be diligent with my best work, perfection is not guaranteed, and mistakes happen. If I was accused of negligence or wrongdoing, I would take full responsibility for my part in the situation. However, I would also defend my expertise and decision-making skills. The best thing any medical professional can do is try their best every day, document their actions clearly, and choose to work with colleagues they trust."

      "I am aware of cases of pharmacy malpractice. Among the local Vietnamese community, there is a physician who just writes up prescriptions for whatever his patients ask for without his first consulting and determining if the medication is right for them. I believe that he has been investigated in the past but was cleared of any suspicion. If I was faced with pharmacy malpractice or negligence, then I would take responsibility but also defend my actions because my decision made in what I believed, as a pharmacist, was in the patient's best interests."
