On occasion, mistakes will happen and communication breaks down. However, in most cases, this is easily avoided, depending on the skills of the individual. Conveying the big picture, the stakes involved, and consistently following through are integral to functional communication. Yet, the message sent is not always the message received, and although an employee might confirm they understand their instructions they may yet either fall short of the expectations which were inadequately communicated, or may fail to communicate they've completed the task without understanding the stakes involved. Yet the person who is receiving the information is just as responsible, especially if they are a stronger communicator than the one sending the message. Something as simple as paraphrasing or mirroring can easily ensure things go according to plan. Consider these points when formulating your answer.

"I recall one instance where I misread an email from one of my subordinates. The punctuation was incorrect and I read it in a dire tone rather than in a positive one, as it was intended. After a tense conversation, the employee clarified her intention and it seems I was mistaken. She and I agreed to exercise verbal communication on a regular basis to avoid a similar situation from happening again."

I was really upset, because I try my best at all times, and felt that the conversation would have been better delivered to me in-person. The next day, I approached the manager and requested that future communication of that nature be conducted in person, versus text message. I did apologize for my initially defensive reaction."
