I have done both clinical and management positions throughout my career, but I feel I need to expand my scope a little further. Having been a Rehab Director & Rehab Coordinator in the past has given great tools and experience. Those positions are always combined with doing patient care, which I love, but I have done patient care for such a long time now, 27 years, and I would like to expand my role in a management position.

You have some well thought out goals, which is excellent. Try starting off with a clearer goals statement and more specifics. I have reworded slightly, below.
"I would like to expand my role into a more significant management position such as (specific job title or function). I have done both clinical and management positions throughout my career, and feel I need to expand my scope a little further by including (A), (B), and (C) responsibilities. Having been a Rehab Director & Rehab Coordinator in the past has given great tools and experience. Those positions combined management with patient care, which I love, but I have done patient care for 27 years, so it is time that I focus further on management responsibilities."
