I rate myself as 9 or 10 because I find myself not to be as clear as I want to be on occasion. I keep an open door approach with my crew and they know to ask for clarification if there is any ambiguity in my communication.

A rating of a 9 or 10 is high and indicates that you are a great communicator but right after giving this rating you state that you're not always as clear as you want to be (which counteracts the high rating!) Consider rephrasing your response as seen below (and don't forget to add examples to your answer as well):
In general, I'd rate myself as a 9 because I'm a very strong communicator. To break that rating down a bit, though, I would say that I'm an 8 when it comes to verbal communication, a 9 for written communication, and a 10 for listening. Because I'm not always as clear as I intend to be verbally, I maintain an open-door approach with my team, and they know they can ask me to clarify what I mean at any time without any backlash. I prefer they ask and be 100% clear on what I meant. I continually work to improve my communication skills in all areas because I recognize how important it is in this profession to be a great communicator whether with my team, patients, or administrators.
