Without proper documentation, you cannot prove that something was said or done. Describe to the interviewer how you rate your documentation skills and why you gave yourself that rating. If you feel you have poor documentation skills, you do not necessarily want to tell that to the interviewer, but you should have a plan to improve your skills.
