"Prior to my current position, I was a retail pharmacist and a major part of my job was reviewing medication orders, identifying medication problems, and resolving them. Also, I accurately dispensed compound medication as ordered by physicians. Since I worked at one of the busiest pharmacies in the city, I had experience supervising 5 technicians at any given time. In my current position as a medication therapy management pharmacist, I was able to provide counseling for each patient which gave me the opportunity to help them improve the way they manage their health conditions. By completing each review, I gained experience identifying and resolving any therapy-related problems. Completing comprehensive medication reviews with the medical staff and especially nurses has given me the opportunity to provide them with drug information and education. I often communicate clinical recommendations or interventions with prescribers especially when discrepancies are identified in the patient's therapy.
I also complete plenty of Target-ed Intervention Programs (or as known as TIPs) so I have the ability and the experience to consult physicians, interpret clinical guideline criteria for them, and advise them of suitable formulary alternatives when needed.
Although transitioning from an outpatient to an inpatient setting may have a bit of a knowledge gap, I am very excited about the challenge. I am quick to learn new things. I'm also good at using resources to figure things out on my own. I will do whatever it takes to fill that knowledge gap as quickly as possible, even if that meant improving my clinical knowledge during my personal time. I believe I have all the skills needed to not only meet but also exceed the expectations for this position."

During pharmacy school, I successfully completed 6 courses in pharmacotherapy and 2 courses about dose optimization. These two types of courses helped me develop a number of skills, including understanding the influence of dosage forms, dosing regimens, and dose levels. I gained an understanding of the relationship between drug concentration, effect, and side effects. I also learned about designing optimized dosing regimens for patient care, utilizing drug monitoring techniques and computer technology, and applying the principles for pharmacokinetic decision-making to improve patient care. I also used clinical reasoning to workup the patient and make evidence-based therapy decisions to solve patient problems. I am able to write a SOAP note that accurately and concisely summarizes the patient's workup. I also learned to communicate with patients and other health professionals during the development and implementation of the care plan.
I also successfully completed 3 required APPE rotations of general hospital practice and general medicine 1 and 2. These rotations gave me the opportunity to participate in patient care and interprofessional care team rounds. I also participate in therapeutic drug monitoring activities by applying pharmacokinetic dosing principles to drugs such as antibiotics and recommending dose adjustments for renal and hepatic impairment. I was also able to prepare formal SOAP notes and complete a few written drug information responses using appropriate institutional documentation.
Although I am not currently working in an inpatient setting, I have completed a critical care pharmacy review course that was offered by ASHP which was about 25 hours of credit. This course refreshed my memory on topics such as pneumonia, sepsis, acute coronary syndrome, hyperglycemic emergency and community-acquired pneumonia, oncologic emergencies, GI bleeding, and acid-base disorders, infectious diseases in critical care patients, toxicology, and more. I also obtained pharmacogenomics certifications through RXGenomix.
Generally speaking, if there is anything that I may have forgotten, it won't take me long to remember and become comfortable with it. I also plan to review and refresh my memory with some basic hospital knowledge prior to starting this position, such as antibiotic coverage, anticoagulation basis such as bridging, IV anticoagulants and reversal agents, and insulin dosing. I also plan to review medications such as vasopressors and inotropes, tPAs, vasodilators & IV antihypertensives."

To make sure I am well-informed, I subscribe to a variety of trade publications and newsletters such as APHA Pharmacy today, ASHP newsletters, and the Florida department of health updates and I receive emails daily from these sites. I find the information to be fresh and valuable. In my spare time, I visit the PubMed site to check out the latest literature and trending articles. I sometimes also use the Merk Manual to also check out the latest news and refresh my memory with certain topics and I also use it to test my knowledge with their case-based questions and quizzes. Finally, I believe continuing education isn't just a requirement, but an opportunity to ensure any knowledge gaps are filled, including in regard to emerging treatment options. And Although I am not currently working in an inpatient setting, last year I completed a 25-hour review course in Critical Care Pharmacy that was offered by ASHP. I also obtained my pharmacogenomics certification through RXGenomix back in December of 2019."
