Prior to my current position, I was a retail pharmacist and a major part of my job was reviewing medication orders, identifying medication problems, and resolving them. Also, I accurately dispensed compound medication as ordered by physicians. Since I worked at one of the busiest pharmacies in the city, I had experience supervising 5 technicians at any given time. In my current position as a medication therapy management pharmacist, I was able to provide counseling for each patient which gave me the opportunity to help them improve the way they manage their health conditions. By completing each review, I gained experience identifying and resolving any therapy-related problems. Completing comprehensive medication reviews with the medical staff and especially nurses has given me the opportunity to provide them with drug information and education. I often communicate clinical recommendations or interventions with prescribers especially when discrepancies are identified in the patient's therapy.
I also complete plenty of Target-ed Intervention Programs (or as known as TIPs) so I have the ability and the experience to consult physicians, interpret clinical guideline criteria for them, and advise them of suitable formulary alternatives when needed.
Although transitioning from an outpatient to an inpatient setting may have a bit of a knowledge gap, I am very excited about the challenge. I am quick to learn new things. I'm also good at using resources to figure things out on my own. I will do whatever it takes to fill that knowledge gap as quickly as possible, even if that meant improving my clinical knowledge during my personal time. I believe I have all the skills needed to not only meet but also exceed the expectations for this position.
