Transparency is always a good idea, and if something went wrong, then it needs to be addressed to prevent it from happening again. When it comes to clients, I will apologize on behalf of the company for errors. In that case, I will do what it takes to protect the company's reputation."

I don't know if it would be considered taking the fall for someone, but recently my new boss gave a project to a co-worker. I needed to show him where he could find the information that he needed because this project was not part of his normal duties. I ended up helping him a few times during the project. After he completed it, my boss asked who did the project, me or him? He did it, but I had to help him more than my boss wanted me to. But I downplayed how much I helped because I didn't want him getting into trouble.
