As a Private Tutor, how do you measure your students' progress and address learning obstacles?

"I measure progress by tracking objectives and standards that I've taught, and I do this weekly. After I teach a skill, I give students a formative assessment to determine their comfortability with it. I keep track of each standard in a spreadsheet and write notes after each session so I can always refer back to what was addressed at a specific session and how it went. When there are obstacles, I do my best to adjust my approach and cater to each students' unique learning style until they have a good grasp."
