It is not lost on me how reimbursement by third-party payers can be affected by incomplete charts. Just as important is the need for accuracy, and attempting to remember specifics from a procedure or rounds can become spotty if too much time has passed. For these reasons, I agree that keeping current is important and would always strive to be timely in my charting as a GI Fellow in training."
