"I had a co-worker who consistently seemed sleep-deprived, was suddenly showing up to work late, and their attention to detail wasn't what it once was. I tried to talk to them about it, but despite my approach, they were increasingly defensive and weren't forthcoming about what was going on. It turned they fell prey to some substance abuse problems, starting from some opioids they'd been prescribed for an injury at work a while back. They took advantage of this service, and after a few weeks off, they returned to work and were back to their old self."

The assessment vendor will evaluate your reasoning for bringing attention to the situation, and your approach. Situations like this can be sensitive and may affect how others on your team perceive you. What you need to communicate here are your intentions. Are you singling someone out to make yourself look better, or are you addressing a coworker who is struggling, trying to support each and every member of your team?

Therefore, you want to come across as confident and relaxed. Avoid any defensive behavior, posturing, movements, or gestures that could be interpreted as insecure, closed-off, unapproachable, hostile, or disinterested.
