Back in the days of my job in the local health insurance, I have to participate in the high-risk tool development. I did a lot of research and come with a useful tool and examples to make it complete as the CMS requirements. Also, I worked very hard in the creation of the individualized plan of care according to the disease.

Good answer! Is the individualized plan of care separate from the high-risk tool development? I suggest sticking to one example (perhaps the high-risk tool development) and expanding more on what you did. I would expand a little more on the actions you took throughout the project, and also describe the impact or result. What the interviewer is looking for here, in addition to experience leading a project, is what type of impact you've made.
