The best way to approach this is to show that you would own up to the situation while doing everything required to fix the potential issue.

"I keep checklists to help ensure I've followed every procedure. If I accidentally do something wrong at work, I will immediately fix the error and fill my supervisor in, if required. If the situation were more serious and required an outside department to help, I would ask for assistance without hesitation. The longer a problem sits, the worse it can become, so I believe it's best to own up and fix the issue immediately."

"I am not afraid to ask for help. If I accidentally did something wrong at work, I would let my manager know immediately. I would also ask for extra training to keep the situation from happening again. Prevention is key!"
