"Last year, I was the care manager for a patient who was in a very difficult health and financial situation, compounded by the fact that they didn't have any immediate family or caregivers in the area. I worked diligently to help find immediate wound care treatments for him and this included an initial hospitalization. I communicated closely with the physician and care team to get an idea of when he would be discharged. Knowing that being in his own home was important to him, I made several calls to get in-home care set up for him on a daily basis. Being that his coverage was limited, I had to be very thorough in finding care at the right cost and the right services. To this day, he is still doing very well in his home as I observe on the monthly call to his home. He always brings a smile to my face and I'm looking to bring this same level of service to your team and members at UnitedHealth Group."
