"Working as the director at our Palliative Care department, I often get consulted from our physicians in other specialties on if palliative services are right for a particular patient. Recently, I was asked to give insight to our home health team on a patient diagnosed with severe COPD who wanted to remain in their home instead of being hospitalized. To gain some background, I met with the patient's primary care provider and home health team to gain some insight. Following that, I joined a couple of home health nurses on a home visit with our Palliative physician to determine if our services would greatly help him and his family."
