For this question, give a specific example of a time you saw this at work for a patient and talk about what role you played and how it helped your patient.
"During my time as a behavioral health nurse, I've seen many great community programs help our patients. Addiction management programs and community integration programs have been extremely useful. With a recent patient, I led the charge to get a heroin addict help with further treatment outside of our facility after his discharge. A quick call to Community Recovery Resources and the patient's willingness made it easy for me to connect him to a treatment program that hopefully yields great results as he looks to piece his life together."
