How would you handle a staff member, who was clearly under performing?

"A graphic designer who worked for me was not meeting his goals and submitting very low-quality work. After a few incidents, I had a conversation with him and asked him what the issue was. It turns out that he was having medical issues, and he was scared of telling me because he thought that I would replace him. I told him to take a few weeks off and come back when he felt up for it. When he did come back, he performed well and turned in all projects on time. We still work together on major projects."
