Focus on assuring the interviewer you have top-notch documentation and communication skills. You do not necessarily want to rate yourself a 10, as there is always room for improvement. However, the interviewer is looking for team members who would rate themselves on the higher end. If you have taken a communication or documentation course to improve your skills, highlight that in your response.

"I would like to rate myself a 10, but I know there is always room for improvement. So, I would rate myself a solid 9 in my documentation and communication skills. I took a medical documentation course and two semesters of communications in college. Those courses set me up for excellence and taught me how to speak to diverse groups, communicate through written means, and document patient findings to ensure accuracy. I have never had a complaint about my communication and documentation skills and have been commended in past employee performance reviews for these skills."
