The difference among agendas only increases when more competing factions take part within a bureaucracy. Competition can be among profit-making individuals/practices on the hospital staff who are fighting for market share or even for recognition and kudos pitting peers as part of a single group against each other. The fact that bureaucracy means lots of meetings, it doesn't help that you might have to withstand the uproar of battles that aren't even relevant to you.
