"At my previous job, one of my client's records went missing. The last person who handled it was my colleague, but she had no idea where it was. She became slightly hostile towards me. As the papers contained confidential information, it was a serious concern for our team. Instead of pointing fingers, my coworker and I broached the subject with our boss. We searched the entire office, and the file in question was found at someone else's table. It turns out that it was mistaken for another document.
