iConnect Health Center classes are designed with patients who suffer from chronic conditions in mind. Statistically, over 60% of clients who suffer from one chronic illness will often develop another. The goal of Fraser Health Authority is to help patients manage their current condition and prevent any additional diagnosis from developing. Some classes include: Healthy Eating Series,
Weight Management, Stress Management, Managing Blood Pressure, Managing Cholesterol and Medication Management. While experience working with chronic conditions may not be necessary for the job for which you are applying, this is a good time to share any experience you do have with the interviewer.

"I actually have experience working with both chronic and acute patients. I worked in the emergency department for a few years which, obviously, was acute care. I also worked in a long-term care facility where the majority of the clients had at least one chronic condition that was managed. I love the idea of having classes to educate patients regarding things that they can do independently to help manage their health. iConnect Health Center sounds like a great opportunity for patients to learn and grow."

"The majority of my experience has been in acute care. However, although I worked in acute care, many of our patients came in due to exacerbations or complications related to a chronic condition. Although acute and chronic illnesses are very different, many are very closely related to one another. I would love to visit the iConnect Health Center. It sounds like a great place for patients to feel like they can take an active role in their own care."
