"I entered medical school intending to pursue a career as a pulmonologist, and spending the last three years in an amazing internal medicine residency program confirmed that this is the right path for me. After my fellowship training, I want to practice in an academic setting where I can treat patients, educate medical students, and pursue research in clinical trials."

Put thought into how you explain your career goals as a physician, and outline how the fellowship program you are interviewing with will help you reach those goals. It is also okay to highlight several goals and say that you are still considering your options. Remember that you do not want to shut the door on any potential opportunities.

I entered residency with the goal of going into pulmonary and critical care medicine. The time that I have spent training at AJH confirmed that this is the right path for me. I have always had an interest in teaching. During my undergraduate, I worked as a tutor assigned to a class and also did 1:1 tutoring. After my fellowship training, I want to be in a setting where I can continue to teach; whether to medical students, residents, or fellows, alongside my clinical duties.

Nice! You come across as ambitious and clear about what you want to achieve. It's also great that you support your assertion about having an interest in teaching with an example of some times when you've taught in the past. Good response!
