Several members of the team did sign off but a few of us really did not feel that backdating and signing off on training that was not completed was an ethical approach. A few of us ended up asking to meet with the Director and we reported this request."

"In a prior role, I made a mistake with my documentation while recording sample information. I mistakingly miswrote a number that was not accurate. When I realized my mistake, the results had already been handed over to the client. I contacted the client to let him know that my mistake had invalidated the testing that was completed for him and that the testing would need to be rerun. He was incredibly understanding and patient. Going forward, I have always double and even triple checked my work to avoid repeating that error."
