They inspired me to study hard through high school, and I earned scholarships to pay for my undergraduate degree in pre-medicine."

I received feedback on a paper early in my undergraduate program that said I wasn't detailed enough in my work. Rather than get upset by this, I thanked my professor and told her I would improve the next time. Here at Wayne State University, I'd look forward to working with and learning from your world-class faculty to help me become the best MD I can be."

I deliver full-spectrum reproductive and sexual health care for a diverse number of patients. During one visit, I had a difficult time counseling a female patient who was seeking pregnancy due to my implicit biases about her young age. I inquired heavily about her understanding of pregnancy and family planning and started to unconsciously tailor my counseling towards family planning. However, she confronted my bias and disclosed that she was seeking pregnancy at a young age due to a history of infertility in her family. I then realized that she was extremely well informed about pregnancy and birth. This caused me to reflect on my perspective and change my counseling so I could provide her with nonjudgemental, holistic, and safe care. I now push myself to constantly evaluate and mitigate my implicit biases so I can ensure to deliver high quality, patient-centered care."

How do you respond to setbacks? Are you and your work affected?

"Experiencing a setback is always disappointing as a student and can be very disheartening, especially when involving a project that I have put a lot of time and effort into. I understand that setbacks happen to medical students, despite their level of expertise or years of experience. If I experience a major setback, I will take a few moments to debrief internally and take the time to learn from the situation. Your amazing faculty at Wayne State University would be a great team to learn from."

Thus, I would allow myself to feel the feeling, make a plan for self-care, take a break from the project/task, and then regroup to find what went wrong so I could use this opportunity to learn and grow. I believe that every setback is an opportunity to grow."

As a physician, you will be faced with death on a daily basis. How will you professionally cope with this reality?

"I recently read a paper by Dr. Lea Baider and Dr. Simon Wein on the realities of physicians facing death regularly. The paper suggested that physicians who successfully deal with death at appropriate times do so by compartmentalizing their day's events. The paper went on to say that if a physician is to split the mechanics of the problem from the emotional side, the physician can continue to function without the emotions getting in the way. That is an excellent recommendation, but it is also important to choose an appropriate time to address those emotions. This would perhaps mean having a mentor or counselor I could visit regularly. Also, not internalizing a death or blaming myself for the event."

For many students, the four years of medical school training will be their first time facing many of the harsh realities of the profession. However, in the Nepali culture, death is as special as birth and is culturally celebrated, which gives me particular coping skills to deal with death as a part of life. I also have dealt with death personally, so I am well acquainted with grief and have my coping strategies in place which involve a strong network, community, and therapeutic art/dance as well as speaking to a counselor if necessary. Additionally, as a part of my birth work training as a doula, I was also trained for death work because the first thing we are taught in birth work is that death is as much a part of life as birth is."

"I think it is important to take time to reflect in a personal sense on a routine basis in order to process any feelings that arise with these circumstances. It is difficult because you must present an empathetic and neutral front with patients and families, however, I think it is essential to take time for yourself and seek out resources if necessary such as support groups, possibly even specifically for physicians and other healthcare workers/providers."

How will you express your opinions on controversial topics such as abortion, euthanasia, physician-assisted suicide, and cloning as a medical student?

"When it comes to controversial topics such as abortion, euthanasia, physician-assisted suicide, and cloning, I will give my opinions more freely in a social setting with friends versus the workplace or around patients. At work, I would try to keep my personal opinion to myself or, at the very least, remain diplomatic by expressing how each side has their right to an opinion. That can often provide great dialog with patients, and I look forward to having those conversations."

The medical field deals with many controversial topics that people have varying opinions on. As a medical student, you'll need to remain tactful and knowledgeable at the same time on all of these topics. I believe in individuals' freedom of thought and choice, so I would never try to enforce my beliefs on others but would stand up for my beliefs."

What is your opinion on socialized health care versus privatized health care?

"I believe that the basic concepts of socialized health care and privatized health care are good by nature and speak to a range of patients. Some of my peers have expressed their love for socialized health care because, although they pay a higher tax rate, they are not caught in an expensive whirlwind when they face illness. On the other hand, privatized health care can offer significantly shorter wait times, which is very crucial for those with a potentially critical or debilitating illness. I believe it is important to have options for both so people can choose."

As a medical student at Wayne State University, you will be exposed to the business and financial sides of medical practice. This question is meant to start a meaningful conversation between yourself and the interview panel. They want to see a bit of your personal opinion, critical thinking skills, and ability to be diplomatic when discussing socialized and privatized health care.

I believe that if the goal is increased access to quality and comprehensive health care then the mechanism is not as significant, as long as the goal is reached."

What are your thoughts on alternative medicine? Which aspects do you agree and disagree with?

"Although I support traditional medicine, I understand that alternative medicine can greatly benefit some. I agree that there is a science behind alternative medicine; however, alternative medicine should be more strictly regulated. People need to educate themselves better before jumping into 'all-natural' therapy. Prescribed teas and herbs can be dangerous if taken in the wrong dosages. Even though it is natural, it doesn't mean your body will like it. As with all medicine and practices, research and education are essential."

Alternative medicine and non-traditional treatments for many diseases are becoming more popular over time. With this question, your interviewers are hoping to hear that you have some knowledge of alternative medicine and to hear your thoughts on the subject.

"My primary reason for becoming a physician is to provide world-class care to patients, so I feel that the overall delivery of healthcare can become more patient-centered. Great strides are already being made in putting patients first with great online tools, but I think we can do much more! Providing top-notch educational materials to patients and making on-demand in-home care available are areas I'd love to focus on in the future on the administrative side."

I believe that healthcare should progress towards this integrative approach, where physicians educate patients on critical social determinants of health and treat patients inside and outside of the exam room."

What research did you conduct on our medical school before applying to Wayne State?

"I researched Wayne State University a lot before applying. I started by interviewing some alumni I know, and I also spoke with a couple of your current students in the medical program. Last year, I attended the open house here as well. Your school left a positive impression on me because of the great curriculum, faculty, and research opportunities, which is why I chose to apply here."

"Although I support animal rights, I understand that animal testing and research are still required for pharmaceutical testing and many other research platforms. I took some time to read about Wayne State University's division of laboratory animal resources on your campus and was glad to see that many regulatory and compliance efforts are in place to ensure that animal testing is done humanely on your campus. As a medical student here, I'd love to learn more about the great efforts on your campus."

As a future physician, how do you plan to use your role to benefit members of your community? Do you think volunteering is important?

"Bettering my community is going to be a big part of what I do as a physician. I already spend much of my time volunteering at the local children's hospital, and I plan to continue my volunteer work after medical school. One specific way I will help my community members is by holding free educational workshops every month. The workshops will tackle tough issues that the community is facing, such as narcotics abuse and increased teen suicide rates. Wayne State's reputation for serving the Detroit area is one factor that drew me to your program, and I look forward to helping give back as a medical student here."

I would continue my current work as a menstrual advocate and hold free workshops for the community on menstrual education, health, as well as safer sex courses. I would also want to partner with local NGOs to host drives for menstrual products and conduct free STI testing and HIV testing."

"I think engaging and benefiting community members and volunteering is essential. It is our job as physicians to work with not only individual patients but community members to forge trusting relationships with them and make sure that we are doing all that we can to improve the health of populations at large. I think volunteering is important for us to introspect as well as serve others which is the core of the physician's profession."

If you could meet anyone in the history of medicine, who would you choose and why?

"I would love to go back to meet Edward Jenner and explore more of his hands-on work in creating the first smallpox vaccine. His work in immunology and immunizations was revolutionary, and meeting this pioneer and learning his thought process would inspire me."

"I have never been a queasy individual, and I fully understand the types of cases I will see in my career as a physician. I'm very excited to begin working with my cadaver during medical school here at Wayne State. I've spent hours volunteering at a drop-in shelter and have seen many situations involving blood. Rather than think about the gore, I focus on the fact that it's a human being who needs my help. I believe this mentality will get me through any gruesome or undesirable situation."

"During my undergraduate program, I read PLOS Magazine and The Lancet regularly. Both are packed with great general science and medical articles and studies. As a medical student, I plan to join the AMSA and AAFP upon enrolling into a program."

For instance, during my biophysical chemistry class this year, I fell behind as we approached the subject of molecular interactions. To ensure my success, I hired a Department of Chemistry tutor. This approach was beneficial, and I gained a much clearer understanding of the subject, which improved my grade. Should I come across any difficulties while attending Wayne State University, I will not hesitate to seek outside assistance."

"I am a well-organized student and have created a plan that works well for me. When I prepare for an exam, I will map out the content I need to learn and divide that content over the number of days I have to prepare. When I create a study calendar, I leave the final three days leading up to the exam to review the entire library of content. I am a big fan of study aids, as they work much better than simply reading and re-reading the materials. Aids such as flashcards are also much more helpful than highlighting my notes. I also create practice problems and, when possible, will pair up with a fellow student to test each other in written form or verbal lightning rounds. My approach has been successful so far, and I'm confident that my method will prepare me for success as a medical student at Wayne State University."

"From what I understand, if the patient is of sound mind and understands the risk they are taking by refusing medical assistance, I do not have the right to override her religious choice. Many factors could change this, such as an unconscious patient, someone under the influence of a narcotic, or a significant language barrier. In those situations, I should refer to my facilities' leaders and ethics community. I would not feel comfortable making a decision like that entirely on my own."

Moral and ethical dilemmas can be a very real part of practicing medicine, and there is a good chance that you will face some of these situations as a medical student at Wayne State University. If the woman was conscious, I would assess/evaluate her ability to make sound decisions and be competent in making healthcare decisions for herself. If, after knowing the risks, she decides not to undergo it, it is not my position to override her religious beliefs but still do my best to provide her the best quality care. I would examine alternative options with her to see if there was something else we could do to help. However, if the woman was unconscious or in a mental state not allowing her to make sound decisions, I would have to consult the ethics committee or my colleagues of the hospital before making a decision."

How would you react if you discovered a classmate cheating?

"An honorable person will do what is right, even when nobody is looking. The medical industry deserves only honorable graduates. I would first approach my classmate and encourage them to admit their cheating to our professor. If they were hesitant, I would approach a faculty member here at Wayne State to inform them."

Although there is support for this idea, it should not be exercised unless the case is extreme. Physicians have a fiduciary duty to their patients, which must be taken seriously. Honesty is always the best policy when discussing matters with patients."
